For Providers

OBH now manages provider information, applications, modifications, and renewals through our LADDERS site.

Existing Providers

If you currently hold a state issued license or designation, please email the LADDERS support team at CDHS_LADDERS@state.co.us to request access to LADDERS. Please include the following in your email:

• Current agency name
• License number (for CS and SUD)
• Your contact information 

Existing Providers with a username can login at https://socgov06.force.com/LADDERS/login.

New Providers

Please submit agency information through an online form here: 
https://colorado.gov/LADDERS/OBH_ProviderRegistration

Upon OBH verification, the provider will receive a username and access to the OBH Online Application System (LADDERS).
