Care Coordination Subcommittee

Community Living Advisory Group Report Published:

On September 30, 2014, the Community Living Advisory Group submitted its Final Report to the Governor and the Executive Directors of the Colorado Departments of Health Care Policy and Financing, and Human Services.  This report included recommendations related to care coordination.  For more information, please see the Community Living Advisory Group "Final Recommendations Report and Resources" Webpage.  The Department thanks the Care Coordination Subcommittee Co-Chairs and Members for their hard work and success!


The Care Coordination Subcommittee reviews existing care coordination efforts and programs and makes recommendations regarding how to make care coordination more effective.  Works in concert with Entry Point/Eligibility Subcommittee and across other Department initiatives including Colorado Choice Transitions (CCT), Medicaid Expansion Project, Regional Care Collaboration Organization (RCCO) development, etc.

Focus Areas:

  • Duplication and gaps in care coordination services

  • Building on effective care coordination efforts

  • Independence and training for care coordinators

  • Case load

  • Flexibility of care planning

  • Care Coordination in transitional situations

As of March 2013, this subcommittee has been meeting on the 4th Friday of the month, 9:30 am - Noon.

The subcommittee meeting for September 26, 2014 has been canceled. The subcommittee recommendations have been completed and submitted to the CLAG!
