Plan Options

Copay Choice Plus Plan with a Deductible

With this plan, you can use any doctor, clinic, hospital or health care facility you want. You save money when you use the national network. You also have coverage if you use out-of-network providers, with a separate deductible and coinsurance amount.

How the plan works

  • You'll usually pay a copayment for doctor visits and prescriptions.

  • You'll usually have to pay a deductible before your plan will pay for covered services.

  • If you reach the deductible, you'll have coinsurance, where you share a percentage of the cost with your plan.

  • You are protected with an out-of-pocket limit. You'll never pay more than your out-of-pocket limit during the plan year. The out-of-pocket limit includes all of your network payments.

Note: You won't need to worry about these costs for preventive care if you stay in the network.

Plan Highlights

Get more protection with a national network and out-of-network coverage.

  • There’s coverage if you need to go out of the network. Choose what’s best for you. Just remember, out-of-network providers will likely charge you more.

  • There’s no need to choose a primary care physician (PCP) or get referrals to see a specialist. Consider a PCP; they can be helpful in managing your care.

  • Your preventive care is covered 100% in the network. You don’t have to pay any out-of-pocket costs for preventive care as long as you use a network doctor.

High Deductible Health Plan with HSA

You can use any doctor, clinic, hospital or health care facility you want. You save money when you use the national network. You also have coverage if you go out of network.

How the plan works

  • You'll usually pay the full cost of services until you meet your deductible. However, a Health Savings Account (HSA) helps you fund your deductible.

  • After your deductible is met, you'll pay coinsurance for most visits and services.

  • You are protected with an out-of-pocket limit. You'll never pay more than your out-of-pocket limit during the plan year. The out-of-pocket limit includes all of your network payments.
