Request for Application Information

Published Date: May 7, 2018

The Colorado Division of Disability Determination Services (DDS) is a 100% federally funded program that makes determinations of medical eligibility for Social Security Disability Insurance and Supplemental Security Income for the Social Security Administration in the State of Colorado. DDS is seeking qualified* physicians, psychologists and other medical providers in specific geographic areas of Colorado to perform consultative examinations (CE's) for DDS Social Security Disability claimants.  Providers must be licensed through Colorado Department of Regulatory Agencies (DORA) and have no current disciplinary actions.

DDS is also seeking qualified onsite language interpreters to assist claimants with their consultative examinations.

Request for Application, DDS requirements, fee schedule, medical protocols, and forms can be obtained by contacting LaVonne Mercure, DDS Professional Relations Supervisor, at 303-752-5676 or lavonne.mercure@ssa.gov.

DDS may, at its discretion, temporarily cease to accept new applications in a specific  medical specialty, a specific geographic zone, and/or a specific county in a geographic zone, if, in the opinion of DDS, the number of applications on hand are sufficient to meet its current and anticipated business needs. Providers shall be informed of such moratoriums, and/or removal of any moratorium, via the Division of Disability Determination Services webpage.
