Medical Marijuana Registry physicians

Effective Aug. 1, 2017, all medical marijuana physician certifications must be submitted online.
If you provided a patient with the old paper form on or after Aug. 1. you must submit the physician certification online. To do this, log into your account and follow the steps to submit a physician certification.

Recommend medical marijuana
To recommend medical marijuana in Colorado you must:
  • Be an M.D. or D.O. 
  • Be in good standing and licensed to practice medicine in Colorado
  • Have a valid, unrestricted DEA certification 
  • Have an online account to submit certifications  

Recommending for minors

  • Minor applicants must submit two physician certifications
  • Minor applicants with the qualifying condition of Post-Traumatic Stress Disorder must have one certification from a board-certified pediatrician, board-certified family physician, or board-certified child and adolescent psychiatrist. This physician or psychiatrist must attest he or she is on the patient's primary care team and has diagnosed the patient as having a disabling medical condition.

  • You will receive a response within 1-3 business days.
