What is Medical Home
Medical Home is one of eight statewide priorities for the Maternal and Child Health block grant. In addition to the state level work, five local public health agencies (LPHAs) are also working on the medical home priority focused on aligning their HCP care coordination services with the Regional Care Collaborative Organizations (RCCO) care coordination services for children enrolled in Medicaid.

Local Public Health Agencies, CU Medicine, Children’s Hospital Colorado, Colorado Access, Rocky Mountain Health Plans, Health Care Policy and Financing (HCPF), State Innovation Model (SIM), ECHO, Parent to Parent Colorado, Colorado Health Institute (CHI), Department of Family Medicine.
