Medical home priority work and systems development

 
Medical Home is one of eight statewide priorities for the Maternal and Child Health block grant. The medical home effort is focused on improving access to coordinated care for children and youth with specialty health care needs (CYSHCN) and their families through policy and systems improvements. The CYSHCN Section at CDPHE is working on policy development and implementation across three areas of focus: improving coordinated care, enhancing access to pediatric specialty care and increasing access to information and resources. In addition to the state level work, five local public health agencies (LPHAs) are also working on the medical home priority focused on aligning their HCP care coordination services with the Regional Care Collaborative Organizations (RCCO) care coordination services for children enrolled in Medicaid.
