Medical home

 
A medical home is not a building or a place. A medical home is:
  • A philosophy of quality health care that is patient - and family-centered; facilitates communication & coordination between families, patients, physicians, health teams, and health neighborhoods.
    • Health teams and health neighborhoods include medical, mental and oral health care, specialty care, educational services, family supports, community-based services, and more.
  • A strategy to support better care leading to better health, while containing cost: care is received in the right place, at the right time, and in the manner that best suits a patient's needs.
