Linkage To Care

Linkage to care (LTC) is an on-going process where professionals help people with a new HIV-positive diagnosis, or HIV-positive people currently out of care, navigate the early stages of treatment.  
 
LTC coordinators
 
An LTC coordinator connects people with a new HIV-positive diagnosis or HIV-positive people currently not receiving care to the appropriate medical care that they need. LTC coordinators help them understand what their status means for themselves and others, and to commit to a lifestyle that enhances their health and protects that of others.
