Hazardous waste self-certification checklists
Forms must be completed by an employee who’s directly involved with waste management at your facility.
Use the navigation buttons on the form; don’t use your "Back" button as you’ll be taken out of the checklist and your answers won’t be saved.
- After you hit "Submit," you will be re-directed to the Hazardous Waste Management web page. A copy of your completed form will be sent as an attachment to the email address you provide on the last page.
Your certification isn’t complete until you’ve hit the “Submit” button.
Changing your notification
If you need to change your hazardous waste notification form, such as a change in ownership, name of facility, contact information, location or your generator status, please complete and return an updated notification form. Complete the form online, print and sign, then mail to the address provided on the form.
