Hazardous waste self-certification checklists
NOTE: You must use Internet Explorer 9 or higher. Online PDF forms will ONLY work with Adobe Acrobat or the free Adobe Reader. (Get Adobe Reader) They will not work with the default PDF viewer used by Chrome and some Apple browsers.
Don’t use your "Back" button, as you’ll be taken out of the checklist and your answers won’t be saved.
Forms must be completed by an employee who’s directly involved with waste management at your facility.
If you have more than one facility, complete the certification checklist for the facility noted on the postcard or other notification you received.
- Please print a copy of your completed form for your records before hitting the "Submit" button; you won’t be able to print the completed form after you hit "Submit." If you add your email address in the space provided on page 4, a copy of your completed form will be sent to you as an attachment to email.
Your certification isn’t complete until you’ve hit the “Submit” button.
Changing your notification
If you need to change your hazardous waste notification form, such as a change in ownership, name of facility, contact information, location or your generator status, please complete and return an updated Colorado Hazardous Waste Notification Form. Complete the form online, print and sign, then mail to the address provided on the form.
