Children and youth with special health care needs

 
We ensure that children and youth with special health care needs have the opportunity to grow, learn and develop to their highest individual potential. The vision that guides our work is: Thriving families supported by proactive, integrated systems that adapt to meet needs.
 
We work with partners and local public health agencies to connect families to resources, provide care coordination and improve access to pediatric specialty care.
 
Medical Home is one of eight statewide Maternal and Child Health (MCH) priorities. Through this work we improve access to information and referral, coordinated care, and specialty care, for children and youth with special health care needs and their families.
 
Developmental Screening and Referral is one of eight statewide Maternal and Child Health (MCH) priorities. With partners across the state, we work to ensure all young children receive appropriate developmental screening, and if needed appropriate referral to evaluation and services.
 
Recent Colorado and national data, including maternal and child health performance measures, as well as other measures of interest for the Children and Youth with Special Health Care Needs population.
