CIIS health care users

 
Health care providers can use CIIS to check the vaccination status of patients, review recommended vaccines and manage their vaccine inventory. To use CIIS, your clinic(s) must complete enrollment paperwork and create user accounts for each staff member using the registry. All general users are also required to review our confidentiality and security policies and take online training before using CIIS.
 
Enroll your individual clinic or medical group in CIIS
  1.   Enroll your individual clinic or medical group in the Colorado Public Health Reporting portal (CoPHR) under the appropriate entity type.
  2. If you enroll as an individual clinic, then complete the Clinic Letter of Agreement (LOA) and CIIS Administrator Form links located in the Usage Agreements widget. If you enroll as a medical group, then review and edit your individual practice locations (under the Provider Sites widget), and complete the Medical Group Letter of Agreement (LOA) and CIIS Administrator Form links located in the Usage Agreements widget.
Create a CIIS user account
  1. Complete the General Users Login Form.
  2. Submit form online.
We will email your username and temporary password once the online form has been submitted and all required paperwork has been completed for your clinic.
