Medical home

A medical home is:
  • A philosophy of care that’s patient- and family-centered; facilitates communication among patients, physicians and facilities; and is open and efficient.
  • A strategy to support better care leading to better health while containing cost.
Why the medical home works
This framework from the Patient-Centered Primary Care Collaborative (PCPCC) can help fellow medical home supporters and advocates explain the benefits and strategies associated with delivering patient-centered primary care. It’s organized according to the five key features of the medical home model:
  • Patient-centered.
  • Comprehensive.
  • Coordinated.
  • Accessible.
  • Committed to quality and safety.
