Air permit self-certification
The final step of the air permitting process, to finalize the permit, is to submit the final approval and self-certification packet. Within 180 days after receiving the Initial Approval Permit, you must certify compliance with the permit requirements to receive a Final Approval Permit. The final approval and self-certification packet must be submitted within 180 days of commencement of operations, or the permit may be revoked.

Form APCD-401 Self-Certification Timeline Table. Operators who have facility-wide permits covering multiple emissions points that have phased construction and different startup dates must submit the Self-Certification Timeline Table each time a new emissions point is certified.
After we issue a construction permit, you must file a Notice of Start-up (NOS) for the associated equipment and complete the self-certification process:
You must submit the Notice of Startup no more than 15 days after the equipment begins to operate. The form must include the specific equipment details that weren't provided with the permit application. This includes the make, model and serial-number information of the emissions source and emissions control equipment.
