Phase 7: Implement, promote and monitor

Use this phase to implement the Public Health Improvement Plan (PHIP) you developed in Phase 6. A working PHIP that is monitored, evaluated, updated and communicated to stakeholders on a regular basis will have the greatest impact on improving the selected priority areas in your community.
This critical phase to puts into action the PHIP action plans. Core Public Health Capability Assessment and Planning requires local public health agencies to “Develop, implement and evaluate a public or community health improvement plan (PHIP/CHIP) that is informed by a comprehensive community health assessment (CHA) and priorities identified by communities, at a minimum every five years.”
