Change is coming to the CHAPS website that has been providing assessment and planning guidance for the past 10 years. The website update will be happening March 19-21, so now is the time to download and save any steps, tools/templates and resources that are important to you.

The Colorado Health Assessment and Planning System (CHAPS) is guidance for local public health agencies and the Colorado Department of Public Health and Environment to carry out ongoing public health assessment and improvement planning. Its standardized approach helps local and state agencies meet the assessment and planning requirements of the Colorado Public Health Act of 2008 (SB 08-194 CRS 25-1-501 et seq.) and the national Public Health Accreditation Board

 
CHAPS provides step-by-step guidance and resources to carry out an 8-phased collaborative community health assessment and a public health improvement planning process on a 5-year cycle.
