Procedure:  

 Victim can request over the phone or letter, we recommend they request the criminal history record, based on name, date of birth and possibly the social security number. We will respond and recommend they get a copy of the record and then prints taken to compare identity. Prints taken here in the DCI office.

You may request Iowa criminal history record information by completing the required forms and submitting them to our office by mail or fax. To obtain the forms, please download both a Request Form and Billing Form below. Each last name submitted requires a separate Request Form with payment for each. However, only one Billing Form is needed when submitting several requests at once.

Law Enforcement Agency:  
Turn Around Time:   2-5 Business Days, excluding mailing time
Successful challenge:   The DCI record is noted that, based on prints, one person has used another's information. If victim's information is compromised again, the alias flag is released anytime that criminal history is requested.
Cost:   Person must pay $15 and get a copy of the criminal history record that displays the information that is being challenged.
