School Health Services Program

The Colorado School Health Services Program is run by the Department of Health Care Policy and Financing and the Colorado Department of Education.

Who Qualifies?

The program serves students who are:

  • Members of Health First Colorado (Colorado's Medicaid Program)
  • In a participating school district or Board of Cooperative Educational Services (BOCES)
  • Under the age of 21
  • Disabled or medically at risk and receiving, or referred for services according to an Individualized Education Program, Individualized Family Services Plan, or other medical plan of care with established medical necessity.

How to Apply

You can apply for Health First Colorado for yourself or your child online, on the phone, in person, or through the mail.

You should check with your school to see if they participate in the School Health Services program. Any public school district, Board of Cooperative Educational Services, or educational institution with students in kindergarten through twelfth grade may participate.

Providers employed or subcontracted by the districts that are registered, certified or licensed may provide services.

What Is Required For The Enrollment Of Program Providers?
  • An assessment of the health needs of students in the district,
  • Community input of the health services to be delivered to students,
  • An approved Local Services Plan completed by the district, and
  • A contract for reimbursement of health services covered by Health First Colorado.
