The purpose of the Accountable Care Collaborative (ACC): Medicare-Medicaid Program stakeholder meetings are to provide important program updates and maintain transparent communication between us and the people involved with or affected by this program.
The Accountable Care Collaborative (ACC): Medicare-Medicaid Program coordinates physical, behavioral and social health needs for Medicare-Medicaid members in an effort to improve their health care experiences. Members in this program can remain with their current providers and continue receiving the same benefits and services.
This program builds on the infrastructure, resources and provider network found in the Accountable Care Collaborative (ACC) Program. The ACC connects members with providers, community and social services to help meet their needs and improve their health outcomes.
