Vaccine exemptions

 
Colorado law requires all students attending Colorado schools and licensed child cares to be vaccinated against certain diseases unless they have a medical or non-medical exemption on file (Colorado Board of Health rule 6 CCR 1009-2). You must file an exemption at each school, or child care the student attends. To protect unvaccinated children, students with an exemption from one or more required vaccines may be kept out of a school or child care during a disease outbreak.
 
Medical exemptions
  • Students with a complete medical exemption form (signed by a medical doctor, doctor of osteopathic medicine, advanced practice nurse or delegated physician's assistant) need to file this form only once unless the student’s information or school changes.
  • You can file a medical exemption form with us to include in the Colorado Immunization Information System (CIIS). Check with your child's school or child care to find out whether it needs a paper copy of the medical exemption form. Or, you can file the form with your child’s school or child care.
  • Exemptions you file with us to include in CIIS are confidential. See CIIS Privacy and Confidentiality.

Non-medical (personal belief or religious) exemptions
  • Parents of students in grades K-12 claiming a non-medical exemption must file one annually. These exemptions expire June 30 each year.
  • Parents of students in preschool or child care must file non-medical exemptions at 2, 4, 6, 12 and 18 months of age. These exemptions expire when the next vaccines are due or when the child enrolls in kindergarten.
  • There are several ways to file a non-medical exemption.
    • File the online non-medical exemption form (below) with us to include in CIIS. Check with your student's school or child care to find out if it needs a paper copy of the exemption form.
    • File our downloadable form with us (see below) to include in CIIS.
    • File our downloadable form (see below) with your student’s school or child care.
    • File a statement of exemption with your student's school or child care. This statement should include: the student's full name, age or date of birth, date the exemption was filed, the immunizations declined, and which type of non-medical exemption is being taken (personal belief or religious).
