The Health Disparities and Community Grant Program (HDCGP) is funded by the Colorado state tobacco tax revenue as mandated by Amendment 35 (A35) and by funds appropriated by the general assembly as mandated by Colorado SB21-181. The program typically conducts its funding cycle according to the state fiscal year (July 1 – June 30). All funding is contingent upon Colorado State Board of Health approval, spending authority of the program and appropriations by the state legislature each year. The Health Equity Commission assists in the review of the grant program.

This is an update to the Request for Applications (RFA) #42396 administered by the Office of Health Equity (OHE), Health Disparities and Community Grant Program (HDCGP). The HDCGP received an unprecedented 190 applications from across Colorado pursuant to RFA #42396. While OHE is thrilled with the tremendous interest in this funding opportunity, the Schedule of Activities indicated in RFA #42396 on pages 13 and 14 will be modified to thoroughly complete the review of the 190 applications, including review and funding recommendations by the Health Equity Commission, and final review and approval by the State Board of Health. As stated in RFA #42396, section C. Submission Instructions: General Information, please continue to check the CDPHE program website for the most updated information.
