Use the eligibility calculator to find out if you are eligible for Colorado’s programs and services to keep residents healthy. Many of these programs are available to those that meet certain requirements, including income requirements. Those income numbers are based on the Federal Poverty Level (FPL).

Eligibility requirements
Family planning resources
FPL = 0-250%: You may be eligible for family planning services on a sliding scale.
Nutrition support
FPL = 100-185%: You may be eligible for a food program for women and children under age five called WIC.
Colorado Health Service Corps clinics
FPL = 0-200%: An outpatient, primary care, dental, or mental health clinic may be eligible to participate in the National Health Service Corps or Colorado Health Service Corps on a sliding scale.
Women’s health screenings
FPL = 0-250%: You may be eligible for cardiovascular screening for women through WISEWOMAN.
FPL = 0-250%: You may be eligible for breast and cervical cancer screening for women through Women’s Wellness Connection.
