These initiatives implement or strengthen QI processes that support measurement, reporting and meaningful use of clinical quality measures for chronic diseases and related risk factors, including cancer screenings, tobacco use, hypertension and diabetes.
 
Health information technology (HIT) and PCMH e-Learning modules
Online training modules for primary care practices in Colorado to use at no cost. These modules provide valuable learning opportunities and Continuing Medical Education (CME) credits. 
 
We are leading an effort to establish a competency-based credentialing program and registry for the state’s rapidly growing patient navigator workforce.
 
Team-based care
These approaches engage non-physician team members, including pharmacists and community health workers, in high blood pressure and/or diabetes management.
