CDC-recognized National Diabetes Prevention Programs
Colorado stakeholders representing community-based organizations, health plans, large employers, health systems and public health are engaged in an action plan to make the U.S. Centers for Disease Control’s National Diabetes Prevention Program easily accessible for the estimated 1 in 3 Colorado adults with prediabetes. This action plan balances affordability and accountability among providers, patients and payers.
 
Goals
  1. Achieve reimbursement for the National Diabetes Prevention Program.
  2. Establish mechanisms to screen, identify and refer people with prediabetes.
  3. Identify and assist community-based organizations in training and the recognition process to provide the National Diabetes Prevention Program.
It’s estimated that more than one-third of Colorado adults and half of all adults age 65 and older have prediabetes. Without lifestyle changes to improve their health, one-third of people with prediabetes will develop type 2 diabetes within five years.

Join the National Diabetes Prevention Program Advisory Group
The group meets from 2 to 3:30 p.m. on the second Wednesday of every other month (February, April, June, August, October, December).

Join the National Diabetes Prevention Program Work Group
The National Diabetes Prevention Program Work Group meets by phone from 10 to 11 a.m. on the third Wednesday of every other month (January, March, May, July, September, November).
  • These meetings are for all organizations offering the National Diabetes Prevention Program in Colorado. We discuss ways to drive referrals to your classes and keep participants in them. This is a time to share resources with one another to increase your success.
