Colorado Health Service Corps contract changes

 
Maternity, paternity or medical leave
If you take an extended leave that causes you to be away from your job for more than 35 days during one contract year, we'll extend your end date by adding the number of days in excess of 35 and amend your contract. For example, if you're away for 60 days during the contract year, we'll add 25 days to your service obligation.
 
To maintain your contract in good standing:
  • Notify us of your planned leave, if possible.
  • Provide us with the start and end dates of your actual leave when you return to work.
  • Sign, have your site sign and return an amended contract.
 
Switch to part-time obligation
You're allowed to switch to a part-time obligation if you have completed at least 18 months of your current contract. We'll double the number of days remaining in your service obligation and amend your contract. To request a change to a part-time obligation, you must:
  • Get a letter of support from your employer acknowledging the change to part time and the extended service obligation.
  • Notify us of the date you'll go part time or reduce your clinical contact hours.
  • Sign, have your site sign and return an amended contract.

Site transfer
If the site is within the same organization:
  • Make sure it's also an approved site.
  • Notify us of your new practice address.
  • We won't need to amend your contract.
If the site is with a different organization:
  • You must request approval from us before changing sites.
  • Requests are reviewed on a case-by-case basis.
  • Transferring sites makes you ineligible for renewal funding.
  • We'll need to amend your contract.
 
Residential address change
If your personal contact information changes, please notify us of the change and submit a W-9 form via email. 
 
Name change
If your name changes due to marriage, please notify us of the change, and submit a W-9 form and a copy of your marriage license via email.
