The Division of Workers’ Compensation is a state agency that regulates and oversees insurance carriers, employers, injured workers, and health care providers. The Division of Workers’ Compensation and the insurance carrier are separate entities.
If you are injured, the first thing you should do is seek emergency care, if it's necessary. If your injury is not an emergency, you must select one medical provider from the Designated Provider list given to you by your employer. You must report your injury to your employer in writing within four working days of your injury. If you are unable to report the injury within four working days, you should still report it to your employer in writing as soon as possible. You should also report your injury to the Division of Workers' Compensation using the Worker's Claim for Compensation form (WC 15). Once you have completed the form, mail or deliver two copies of the form to the Colorado Division of Workers’ Compensation, Customer Service Unit at 633 17th St., Suite 400, Denver, CO 80202-3626. You may also email the form to email@example.com.
