Colorado Department of Public Health & Environment Health Facilities Division 4300 Cherry Creek Drive South Denver , CO   80246-1530 (303)692-2800	Community Residential Homes for Persons with Developmental Disabilities are homes that provide services and support for at least four and no more than eight persons with developmental disabilities. These facilities are operated by service agencies or community-centered boards. Supportive services include meals, laundry, personal care, assistance with medications and protective oversight. Activities and transportation are also provided. Residents medical needs must be overseen by a primary care physician and the facility must furnish required medical services and keep residents' medical records.
