Medical Insurance Poll

Open enrollment will be here before you know it. To aid the Department of Personnel & Administration (DPA) in designing insurance plans for the next plan year (FY11 - starting July 1, 2010), DPA would like to find out why employees chose their current medical options. By completing this three-question poll, employees will be able to let DPA know what is important to them when choosing medical insurance. The department will use this data to plan for FY11.

This poll is only for State of Colorado employees currently enrolled in a medical insurance option offered by the State of Colorado for the FY10 plan year (July 2009 - June 2010).  It is anonymous and your name will not be associated with your answers or the collected data, and the State WILL NOT track computers that generate the responses.
