Work-Life Employee Discount Program
Vendor Application and Agreement

 

All vendors must read the following text, prior to proceeding to the application link at the bottom of the page.

This application and any subsequent approval or disapproval of this application shall not be viewed or used as a state contract. The State of Colorado assumes no obligation or liability and does not endorse any of the products, services, or discount vendors. The Work-Life program reserves the right to revoke, deny, or disapprove any application based on Work-Life Employee Discount Program policy or for any other business reasons. By submitting this application, the vendor is: (1) agreeing to all conditions set forth in this application and the Work-Life Employee Discount Program policy; (2) attesting to the nature of all statements and information provided within this application as factual; (3) affirming that the discount offer is an added value or actual discount on products or services; (4) affirming that the business is legally recognized by the State of Colorado and products or services offered at a discount to state employees are of an appropriate nature and not referenced anywhere within the Work-Life Employee Discount Program policy as restricted or prohibited; and (5) ensuring all proper federal and/or state and industry licensing and certification requirements are met whenever applicable.

If the application is approved, the Work-Life Program agrees to place a link to the vendor Web site or post a one-page document on behalf of the vendor (provided by the vendor), within the Department of Personnel & Administration’s Work-Life Employee Discount Program Web site, for state employees to access. The approval period will be effective for 12 months.   Typically, another application will need to be submitted if the vendor wishes to renew or offer a similar discount, after expiration of the 12-month period. At any time and for any reason the Work-Life program reserves the right to remove or modify any content on its Web site.

If the application is approved, the vendor agrees to:

  • Comply with the Work-Life Employee Discount Program policy.

  • At no time initiate unsolicited contact, in any medium, with state employees at work locations.

  • Handle all customer service functions relating to the discount for state employees and promptly resolve customer complaints.

  • Make the offer available to ALL state employees (please list any and all requested limitations or restrictions on the application).

  • Keep the Work-Life Program Coordinator updated with any potential changes that may affect the discount offer to state employees.

  • Provide a Web site link or one-page document that will have all pertinent discount information, including applicable pricing or examples, contact and customer service information, company name and address or location(s), any employment verification requirements, passwords or account numbers, and any limitations or restrictions that apply to the offer.

The Work-Life Program appreciates your time and effort involved in completing this process. It is the policy of the program to not accept gifts of any value, at any time, from any discount vendor. Please refrain from sending or offering any gifts as they will be not be accepted by the Work-Life Program.
