The following Certification and Affidavit form is required if all the following conditions are present:

  1. Vendor/Contractor provides services, and/or Vendor/Contractor is a sole proprietor, and

  2. A written contract or purchase order is not required under Fiscal Rule 2-2, and

  3. Vendor/Contractor provides services and/or sole proprietor provides services or delivers goods without a written contract or purchase order.

Colorado Employment Verification Law

The Colorado Employment Verification law applies to all public & private employers in Colorado, and is in addition to separate federal I-9 requirements. Employers must comply with the provisions of the law for employees hired on and after January 1, 2007. There are 2 main components: (1) an affirmation requirement, and (2) a requirement to make & retain copies of employee identity and employment eligibility documentation.

Effective January 1, 2007, the Colorado employment verification law, 8-2-122, C.R.S. (HB06S-1017)  requires private and public employers to retain an affirmation and copies of an employee’s identity documents for employment eligibility purposes.
