Medicaid Eligibility
Colorado Medicaid is public health insurance for families, children, pregnant women, persons who are blind or persons with disabilities and the elderly for Colorado residents. In 2012, three new programs were added to Medicaid including the Medicaid Buy-In Programs for People with Disabilities and Adults without Dependent Children.
When you apply for Medicaid, your eligibility will be determined based on the information you put in your application. Your age, family size, income and other information asked for in the application is to be sure you are enrolled in the program that best meets your needs.
Note: Family Medicaid and CHP+ are both public health insurance programs for families, children and pregnant women. When you apply for Medicaid, you are applying for both Family Medicaid and CHP+. You do not need to turn in more than one application for you or your family.

Applicants
If you are pregnant, looking for medical assistance for your family or applying on behalf of a child, please print and complete the Application for Medical Assistance:

NOTE: You may also qualify for Presumptive Eligibility (PE) if you are pregnant or are under the age of 19. Submit the above application to your local PE Site, click here to find the location nearest you. Your application for Medicaid/CHP+ will be forwarded to your local County Department of Human/Social Services on your behalf.
If you are 64 years old or younger and have not been determined disabled by the Social Security Administration, please print and complete ALL of the following forms:
