ACC Client Information

The Accountable Care Collaborative (ACC)

The ACC is Colorado Medicaid’s newest health plan. It connects you to the right doctors, coordinates your care with other health care providers, and helps you find support services in your area. 

In the ACC, you receive the regular Medicaid benefit package. You also belong to a Regional Care Collaborative Organization (RCCO) that helps you get the services you need and coordinates your care.

You have a team supporting you

As an ACC client, you will have a Primary Care Medical Provider (PCMP). Your PCMP is your main health care provider. When you need specialty care, your PCMP will help you find the right specialist to treat you. Your PCMP may also help you find other types of services you need to stay healthy. 

Your PCMP is the head coach for your health team, or medical home. A medical home is not a building, house or hospital, but a team that provides quality and cost-effective health care. A medical home will provide most of your health care services.  

Your medical home can communicate with other people involved with you and your family, like your child’s school or social services. Your medical home may also tell you about special programs to help keep you healthy. These programs may provide health and wellness education. They may also give you information about asthma, heart disease, diabetes and other topics.

You and your PCMP belong to a Regional Care Collaborative Organization (RCCO). The RCCO makes sure everyone involved in your care is working as a team. You are an important member of this team.
