Colorado Medical Home Initiative

The Colorado Medical Home Initiative is a statewide partnership among all stakeholders in Colorado—including families, patients, health and healthcare advocacy organizations, physicians and other professionals who provide healthcare services, the Colorado Department of Public Health and Environment, the Colorado Department of Health Care Policy and Financing, local health agencies, various facilities, hospitals, clinics, and other organizations around the state. In short, the CMHI is an inclusive body of people and organizations committed to ensuring a medical home approach to healthcare in Colorado.


What is a Medical Home Approach to Healthcare?
A medical home approach focuses on patient- and family-centered healthcare in which the lines of communication among patients, physicians, and facilities are open and efficient, with the proven outcome of better care leading to better health and lowering cost.
