Find my child's immunization record?

Vaccinations protect you now and in the future by preventing diseases that can sometimes kill or permanently injure. It is important that you keep track of the shots that you/your child have received.


Immunization registries like the Colorado Immunization Information System are one of the best tools we can use to make sure we all receive the shots we need in order to be protected from harmful diseases. You and your healthcare provider can use CIIS as a powerful tool to ensure that you/your child receive the immunizations you need to stay healthy and happy.

You can now request copies of an immunization record for you or your child by completing a Request to Release Immunization Record Form, having it notarized, and submitting it to the Colorado Immunization Information System, along with a clear photocopy of your driver's license or other secure and verifiable identification.


Before you fill out the form, please call the CIIS Help Desk at 303-692-2437 or 1-888-611-9918 to confirm that the requested record exists in the Colorado Immunization Information System.  Please note that CIIS is a voluntary system and does not contain immunization records for ALL Coloradans.


Note: Incomplete forms, forms without proper ID, and forms that are not notarized will not be accepted.
