To avoid any additional fees you may download and complete one of our online forms below and mail it to us with a check or money order.

Fax or mail an application as directed on one of the following forms:

• Birth Certificate Application (English)
• Birth Certificate Application (Spanish)
   

Proof of your relationship or direct and tangible interest is required. Applicants must submit proof of their relationship or documentation to establish their legal interest in obtaining certificates. (i.e. birth certificates, marriage certificates, certified court orders, insurance polices). Please see the Required Identification/Eligibility Documentation page for more information.
