Supplemental APEN Forms

Supplemental forms must be submitted along with the appropriate General or Specialty Air Pollutant Emission Notice (APEN) form for certain types of sources as shown below. Check to see if a supplemental form applies to your source.

Above Ground Storage Tanks (not including condensate tanks)

Submit this form along with the General or Specialty Air Pollutant Emission Notice (APEN) to report new or modified above-ground storage tanks (ASTs). This form is not required for underground storage tanks located at service stations or condensate tanks. Use one form per storage tank. Please complete all applicable information on the form.

Control Equipment

Submit this form with the General or Specialty Air Pollutant Emission Notice (APEN) to report any control device used to reduce emissions. Use one form per control device. Please complete all applicable information on the form.

Incinerators

Submit this form along with the General Air Pollutant Emission Notice (APEN) to report new incinerators. Use one form per incinerator. Please complete all applicable information on the form.

Municipal Landfills

Submit this form along with the Municipal Solid Waste Landfill APEN for reporting new municipal landfills. Use one form per landfill. Please complete all applicable information on the form.

Sand Blasting Control Plan

Submit this form along with the General or Specialty Air Pollutant Emission Notice (APEN) to report fugitive particulate emissions from sandblasting activities, per Colorado Air Quality Control Commission's Regulation 1.
