Centennial Mental Health Center, through ARRA funds granted to Colorado Department of Human Services has created a Criminal Justice Clinical Specialist Program that will work directly with community corrections and the offender population to efficiently and effectively identify and address transitional issues, streamline the referral to services process for Northeastern Colorado and ex-offenders of the 13th Judicial District, Morgan, Logan, Phillips, Sedgwick, Yuma, Washington and Kit Carson.

 
The “revolving door” has been a concerning issue for Colorado corrections and mental health services.  CMHC believes that full and extensive integration of ex-offender transitional services and mental health assistance is the holistic answer to helping individuals achieve self-sustainability in life, as a productive member of society.

 
Adult residents (18 years and older) with co-occurring mental health and/ or substance abuse issues who are currently a part of the criminal justice system process are eligible for services and assistance with the CJCS.  These services may include:

 

  • Assessment of client’s needs, including follow-up
  • Coordination of services with referring criminal justice agencies
  • Direct transitional case management
  • Referral to individual need services (assessments, group and individual treatment, medication evaluation and consultation, residential services, benefits acquisition, vocational/ occupational training and referrals to other assistance programs/agencies)
  • Provide agency-specific training on working with offenders and coordinating with criminal justice agencies, upon request
