Medical Records

Health Information is maintained and managed by the Medical Records Department, for the benefit of the patient and providers for the Colorado Mental Health Institute at Pueblo. Medical Record Services completes health records after patients are discharged by assembling the records, analyzing records for completeness, accurately coding diagnoses and entering portions of the record in manual and automated systems for statistical purposes. Medical Record Staff verify correct legal status of patients upon admission, changes during stay and upon discharge; monitors legal correspondence received, tracks hearings for staff and patients and is the liaison for CMHIP and Colorado courts. The department is responsible for retrieval of information and release copies of health records for use by caregivers, researchers, patients or their representatives. It is the duty of this department to protect the confidentiality of the information contained in the records through knowledgeable application of pertinent laws, rules and regulations, and collects data for CMHIP quality support programs, Medical Staff Committee functions and clinical pertinence.

The Medical Records Department is open for business from 8:00 am. – 12:00 pm. and from 1:00 pm. – 5:00 pm. Monday through Friday.
