• Registration Agreement-  This agreement must be filled out in order to comply with our security requirements.  We ask that the person that fills out this form be the main security contact for the county, as they will be the person that will give and remove access to Colorado PayPort.  They will also receive occasional application maintenance emails.

• CI Security Agreement - This agreement must be filled out by each individual that will need to have access to the PayPort application.  This includes managers, cashiers, etc.

• Checkout Setup Form -  This form provides the appropriate banking information for each organization.  Please only focus on the areas shaded in yellow.

• Number of Card Readers - Use this form to assess the number of card readers you will need per location.
