Driver's License or ID Number Used for Credit or Check Fraud

If your driver's license or identification card number has been compromised, a new personal identification number (PIN) may help you conduct business. Because motor vehicle records are protected by federal and state privacy laws and are not accessible for credit purposes, it will not prevent or stop credit fraud.

Download and complete the Affidavit of Colorado Driver's License or ID Theft form (DR 2153)
Have your signature notarized
Take (do not mail or fax) the Affidavit form with your police report to any driver's license office, which are listed via this link or on a telephone recording at 303-205-5613
