Licensing

MMED Occupational Licensing

Due to Security Concerns:
If you are a walk-in applicant DO NOT approach or enter the building prior to 8:00 AM
The Division is located in a private commercial building
and you cannot be accommodated prior to that time.

Since we have found that applicants sometimes fail to keep their scheduled appointments, the MMED staff will accommodate walk-in applicants for those abandoned time slots.  The number of available slots will be limited, since the total number of appointments conducted during licensing hours is also limited.  There is no guarantee that the MMED will be able to process any walk-in applicants on any given day.  Walk-in applicants will be accommodated on a first come, first served basis.

The MMED staff cannot begin the process without a completed application form, any necessary attachments required by the application, and the full amount of the fee; Support badges are $75 and Key badges are $250. The MMED cannot accept credit cards, so the fee must be paid in cash or a check made out to the MMED of the Department of Revenue.
