Colorado Medical Home Initiative

The medical home approach in Colorado encompasses, health, behavioral health and oral health, as defined in state statute (C.R.S. § 25.5-1-103).


The Colorado Medical Home Initiative (
www.coloradomedicalhome.com)  is a systems-building effort to promote quality health care for all children in Colorado. The Medical Home Advisory Board brings together a group of more than 40 people representing various agencies, families, hospitals, organizations and policy-makers dedicated to building a sustainable system that delivers quality health care for all children.


The Colorado Medical Home Standards, developed in partnership by the Medical Home Advisory Board, the Colorado Department of Health Care Policy and Financing (Medicaid and CHP+) and the Colorado Department of Public Health, are now required to be met by health and medical practitioners serving children and youth enrolled in Medicaid and Child Health Plan (CHP+).

 The Medical Home approach is recognized and endorsed by various governmental and professional organizations, including but not limited to:
