In November, 2013, the CCT program hosted a site visit with Money Follows the Person (MFP) representitives from Centers for Medicare and Medicaid Services (CMS).  The offical follow up letter provided by, Colorado's MFP Project Director, Todd Wilson, of the CMS central office can be viewed here.   

 

The CCT Provider Directory for approved providers by county and by service has been updated and is posted on the CCT Provider Information tab - April 3, 2014

Program Information

 

Colorado Choice Transitions (CCT), part of the federal Money Follows the Person (MFP) Rebalancing Demonstration is a five year grant program.  The primary goal is facilitating the transition of Medicaid clients from nursing or other long-term care (LTC) facilities to the community using home and community based (HCBS) services and supports.  Services are intended to promote independence, improve the transition process, and support individuals in the community.  Participants of the CCT program will have access to qualified waiver services as well as demonstration services.  They will be enrolled in the program for up to 365 days after which time they will enroll into one of five HCBS waivers so long as they remain Medicaid eligible.

Operational Protocol Version 1.0*

The Operational Protocol is a required document for the Money Follows the Person Demonstration.  It should provide enough information that CMS, other federal or State officials and external stakeholders may use this document to understand the operation of the demonstration.  Version 2.0 of the Operational Protocol coming soon.
