The Centers for Medicare and Medicaid Services (CMS) Innovation Center awarded Colorado and 14 other states with contracts to develop new ways to better coordinate care for full benefit Medicare-Medicaid enrollees.
Goals
Improve care coordination;
Improve client experience;
Improve health outcomes for full Benefit Medicare-Medicaid enrollees; and
Decrease costs associated with unnecessary and duplicative services.
