If the pharmacy would like to report problems with purchasing drugs at or below the State Maximum Allowable Cost (State MAC), please use the form titled Colorado State Maximum Allowable Cost (MAC) Request for Medicaid Reimbursement Review.
Prior to submission of the form, the state recommends pharmacies compare National Drug Codes (NDCs) to other available products to possibly locate a more reasonable price.
