The Life Transitions work group makes informed recommendations to the Medicaid Infrastructure Grant Steering Committee on policy and outreach issues related to the clients of the Medicaid Buy-In Program for Working Adults with Disabilities who are in transition. Transition groups, for the purposes of this work group, include - but are not limited to - those transitioning from childhood to adulthood, those aging out of the program, and those with a newly acquired disability.
