Adult Buy-In
The Medicaid Buy-In Program for Working Adults with Disabilities (Adult Buy-In) will provide adults with disabilities, who earn too much income or have too many resources to qualify for Regular Medicaid, the opportunity to purchase Medicaid. Regular Medicaid benefits include, but are not limited to, office visits, hospitalizations, x-rays, home health services, durable medical equipment, and prescription medications. Clients will pay a monthly premium based on their income.

Any of the following applications may be used to request Medical Assistance through the Adult Buy-In program:
NOTE: You may need to submit the Medicaid Disability Application/Release Form in order to complete the eligibility determination process for the Adult Buy-In program. If you do not already have a disability determination through the Social Security Administration, you will need to submit the following forms with your application for Medical Assistance:
