December 2012 and January 2013 - The Transition Coordinator and Intensive Case Manager PowerPoint training contains important information about the history of CCT, the referral and transition process, risk management, benefits and services and service planning. This document serves a reference guide to assist in learning more information about Colorado Choice Transitions.
December 2013 and January 2013 - The TC/ICM Training Manual document contains the handouts provided at the Transition Coordinator and Intensive Case Manager trainings in December, 2012 and January, 2013. These documents are designed to complement the information provided in the PowerPoint presentation and contain forms and procedural information for facilitating a transition.
February 2013 - To learn about the changes made in the Benefits Utilization System that are unique to CCT, please reference the CCT BUS Changes document.
April 5, 2013 - Transition process for Transition Coordinators and Intensive Case Managers training/Webinar PowerPoint presentation explains in detail the transition process. The forms that accompany this training can be found on the CCT Transition Coordination link.
