Intensive Case Managers must complete the CCT PARs for services requiring prior authorization. Note: All CCT PARs are located in one Excel workbook with a separate tab for each HCBS waiver. Click the CCT PARs link above to access the Excel workbook. All paper PARS are electronically fillable. Please follow all instructions included on all PAR forms.
