CCT Housing Resources

 

This page provides details about housing for Colorado Choice Transition clients and resources for accessing housing. 

 

One of the eligibility requirements for people participating in Colorado Choice Transitions is an agreement to move into qualified housing.  Qualified housing, as defined by the Centers of Medicare and Medicaid (CMS) are:

 

  • A home owned or leased by the client or the client's family member
  • A residence in a community-based setting with no more than four (4) unrelated individuals living together
  • An apartment where the client has their own individual lease, living, sleeping, cooking and bathing areas, and lockable access and egress.  Services cannot be contingent on tenancy within the apartment

Department staff have the ability to verify the social security number of client's who are submitting a HUD application, but do not have a social security card.  You can learn about the process in the DAL SSN verfication form and in the SSN verfication form.
